سابقه و هدف: اضطراب دندانپزشکی یکی ازدلایل مهم پرهیز افراد از درمان دندانپزشکی بوده و در دراز مدت از میزان بهداشت دهان و دندان و نیز کیفیت زندگی فرد می کاهد. ازسوی دیگر رخدادن اضطراب در مراجعان و تکرار آن در معاینه بیماران، بر کارایی دندانپزشکان نیز اثر می گذارد. هدف مطالعه حاضر، بررسی تاثیر ارائه توضیحات درمان بر میزان اضطراب بیماران دارای پالپیت برگشت ناپذیر بود. مواد و روش ها: این مطالعه به روش کارآزمایی بالینی بر روی 96 بیمار مبتلا به پالپیت برگشت ناپذیر دندان های پری مولر فک بالا انجام شد. افراد به صورت رندوم به دو گروه مورد و شاهد تقسیم شدند و معیارهای ورود و خروج از مطالعه رعایت شد. پرسشنامه استاندارد اضطراب دندانپزشکی قبل از مداخله به هر دو گروه داده شد و اطلاعات دموگرافیک ایشان ثبت گردید. دو گروه مورد 15 دقیقه توضیحات مربوط به تشخیص و مراحل درمان داده شد و پس از انجام درمان نیز هر دو گروه پرسشنامه مربوط را پر نمودند و جهت آنالیز آماری از آزمون T-Test و paird T-Testِ استفاده شد. یافته ها: قبل از مداخله نمره اضطراب در گروه کنترل 46/10 و در گروه مداخله 71/11 بود (0/05

Introduction: Teeth with Irreversible Pulpitis usually undergo root canal therapy (RCT). This treatment modality is often considered disadvantageous as it removes vital pulp tissue and weakens the tooth structure. A relatively new concept has risen which suggests vital pulp therapy (VPT) for Irreversible Pulpitis. VPT with calcium enriched mixture (VPT/CEM) has demonstrated favorable treatment outcomes when treating permanent molars with Irreversible Pulpitis. This study aims to compare patient related factors, safety and organizational consideration as parts of health technology assessment (HTA) of the new VPT/CEM biotechnology when compared with RCT.Materials and Methods: Patient related factors were assessed by looking at short- and long-term clinical success; safety related factors were evaluated by a specialist committee and discussion board involved in formulating healthcare policies. Organizational evaluation was performed and the social implications were assessed by estimating the costs, availability, accessibility and acceptability. The impact of VPT/CEM biotechnology was assessed by investigating the incidence of Irreversible Pulpitis and the effect of this treatment on reducing the burden of disease.Results: VPT/CEM biotechnology was deemed feasible and acceptable like RCT; however, it was more successful, accessible, affordable, available and also safer than RCT.Conclusion: When considering socioeconomic implications on oral health status and oral health-related quality of life of VPT/CEM, the novel biotechnology can be more effective and more efficient than RCT in mature permanent molars with Irreversible Pulpitis.

Background/purpose: Mineral Trioxide Aggregate (MTA) has become integral in endodontics, particularly in addressing Irreversible Pulpitis due to its notable biocompatibility and sealing properties. This bibliometrics and scientometric study aims to systematically analyse the literature to identify critical trends, influential publications, and emerging research areas.Materials and methods: Publications were acquired online from the PubMed and Scopus databases on December 7th, 2023. Then, studies were screened based on title and abstract and irrelevant studies were eliminated. The publication output was then analyzed using Google Sheet, R package's Bibliometrix (Biblioshiny), and VOSviewer.Results: From 136 studies, 60 studies were included. The study highlights a substantial annual growth rate of 18.02%. Collaboration emerges as a prominent feature, with an average of 4.78 co-authors per document and 21.67% of collaborations being international, with India standing out as the most prominent contributing country. Recent trends indicate a surge in investigating diverse conditions, exploring advanced material combinations, and a growing interest in decision-making frameworks within endodontic practices. Several topics are still less developed, including other materials, decision-making frameworks, special consideration for specific patient populations, and the role of medical informatics.Conclusion: This study elucidates the dynamic evolution of research priorities in MTA and Irreversible Pulpitis, emphasizing key trends and areas for further exploration and development.

Amitriptyline is one of the most common tricyclic antidepressants (TCAs), which has a binding site on pain sensory nerve fibers near to sodium channel, hence could interact in some degree with receptors of local anesthetics. This study was designed to assess the additional analgesic effects of 2% Amitriptyline local gel administration in Irreversible Pulpitis pain of molars. This study was performed on 56 consented adult patients, who had not enough analgesia after lidocaine nerve block for their tooth Pulpitis pain, as a randomized double blind clinical trial. Patients were treated with 0.2 ml of either 2% amitriptyline or placebo, directly injected into their mandibular molar pulp chamber after they had received 2 routine lidocaine injections. Patients were asked to score their pain as a mark on a 10-cm Visual Analogue Scale (VAS) at different timepoints: 0 (just before gel administration), 1, 3, 5, 7 and 9 minutes after the treatments. Results showed that there was a 92.5% decrease in VAS scores of patients, 9 minutes after amitriptyline administration compared to time 0, while in placebo group this difference was only 13.5%. However, in amitriptyline group, VAS score at all time points was statistically different from time 0 (p<0.01). The overall pain reduction and its trend was much more in Amitriptyline group compared to the placebo (P<0.001). In conclusion, inter-pulp space administration of amitriptyline 2% gel for completing analgesia in Irreversible Pulpitis pain could be effective and useful as a conjunctive therapy to injections of local anesthetics.

Background. The present study evaluated the clinical and radiographic outcomes of Biodentine pulpotomy for 24 months in symptomatic vital mature permanent teeth with caries exposure. Methods. Seventy-three patients with a chief complaint of spontaneous pain in permanent teeth were screened. Finally, 47 mature permanent teeth underwent a Biodentine pulpotomy procedure. Clinical evaluation of 47 teeth was carried out at 1, 3, 6, 9, 12, and 24 months and radiographic evaluations were made at 6, 12, and 24 months. The success of Biodentine pulpotomy was evaluated using Pearson’, s chi-square test. The significance level was determined at P < 0. 05. Results. At 24 months, the clinical and radiographic success rate was 97. 78%, with only one clinical failure at 9 months. Conclusion. The clinical and radiographic success of Biodentine pulpotomy was high (97. 78%). Thus, Biodentine pulpotomy can be an alternative to root canal treatment (RCT) in symptomatic vital mature permanent teeth.
